Cobra Control Services (CCS) provides continuants with monthly COBRA payment coupons to make remitting payment for coverage accurate and easy.

After processing a COBRA (or continuation of coverage) election, CCS mails monthly payment coupons to the continuant. Coupons are provided through the end of the plan year. The coupon contains the address for remitting payment and the dollar amount for the month of coverage. Continuants send payment with the appropriate month’s coupon to ensure proper payment for the month of coverage.
Payments for continuation coverage must be made by check or money order and sent to the address specified on the coupon. Some continuants contact their bank to set up direct payment. Payment for continuation coverage must be submitted by the due date listed on the coupon to avoid coverage termination.
If a continuant misplaces a payment coupon, they can still pay for coverage. To make a payment without a coupon, the continuant should write their reference number on the check or money order and send payment to the address provided in the continuation of coverage letter. Replacement coupons are available upon request.
